Documents related to Grant of MFPO Licence

Check List for list of documents and information therein to be scrutinised at the time of (while granting licence under the MFPO, 1973

1. Form A ( Application for issue/renewal of licence).

  1. The items 1 to 3 are to be filled properly giving full particulars including pin code, telephone number, telegraphic address etc. The name and designation shall appear;

  2. For item 4, the source of meat should be spelt out clearly particularly for sub-items 'b' and 'c' the name(s) and address (es) of slaughter house (s) and shop (s) should be furnished;

  3. For item 5, the names of meat food products should be clearly understood indicating the species of meat and the spellings should be correct. If products are not of common nature detailed composition for those products ( all ingredients) without indicating percentages shall be given;

  4. For item 6, the installed capacity should be given and not presently in production that too, in kilos or tons ( 100 kilos);

  5. The existing licence number in the case of new licences, the licensees, granted to them by local authorities or boiler inspectors, or industrial licence or any other licence and while renewal on MFPO, licence No. and category; and

  6. Total value of meat food products should be productwise in Rs. or thousands.

2.  In the blue print or plan of the factory -

  1. The boundaries on all the sides should be spelt out clearly.

  2. The area of the factory and the entrances to the factory shall be indicated and to the extent possible the plan should be to the scale.

  3. Working rooms' measurements with windows, ventilators and doors' measurements are to be given or to be shown as per the scale; and

  4. The inspecting officer should check all aspects for the correctness.

3.  In the list of equipment -

All equipment used for slaughter ( if applicable ) for storing , deboning, mincing, processing of meat etc. should be listed and not items like fans, coolers, almirah, office tables, chairs.

4.  The no objection certificate or a licence shall be produced from a local authority viz. Panchayat/ municipality / corporation as the meat factory may create public nuisance by way of slaughter of animals or using common drainage or creating pollution due to smell, smoke, filth etc. The certificate or licence should be issued by a local authority and no other certificate should be considered as equivalent under the local authorities act or rule so permits.

5.  The partnership deed/ Affidavit / Memorandum of Articles of Association shall be in line with requirements stipulated for issue of certificate of Authorisation under APGM Act, 1937. The names appearing in this document should tally with what has been indicated in items 1 and 2 of Form 'A' failing which Form 'A' shall be treated as invalid unless they are authenticated by observing proper formalities and copies of such documents duly attested shall be provided by the manufacturer.

6. In the labels -

  1. The requirements stipulated in para 10 of the Fourth Schedule shall be complied and

  2. Actual date on which meat food product is manufactured has to be taken as date of manufacture.

7.  In the authority letter -

  1. The names and addresses of persons nominated by the manufacturer should indicate the powers vested with them like assisting the officers in inspection, collection of samples, packing and despatch etc. and/or to correspond with the Directorate of Marketing and Inspection and/or to sign indemnity letters. Signatures of those persons are to be attested by the authorised person and if different people are vested with different powers, the same shall be indicated.

8. The Medical fitness certificate of workers shall be issued once in every year by a qualified Medical Officer or a registered Medical practitioner with MBBS qualification.

9. The water analysis report should be obtained from a public health laboratory or a well equipped and duly recognised laboratory for quality of water relating to physical, chemical and microbiological parameters and water should be declared portable.

10. In Form 'B' -

    All the particulars are to be filled in and signed by the licensing authority.

11. The inspection report at the time of issue of licence should clearly depict the prevailing conditions in the meat factory by giving descriptive explanation for all the items instead of filling 'yes' or 'no' so that anybody can visualise the conditions prevailing without visiting.

12. All the documents received by the office has to be carried to the meat factory for verification and on verification should certify of having verified the position by attesting on the documents whether they are typed/photo copies.
